This is an extreme form of body piercing that thankfully takes place under a generat anesthetic. It is also exceptionally rare with only a few people willing to do this, I guess trying to avoid a man slaughter charge should it go wrong.
For this most extreme form of piercing a drill is used in place of a needle and small holes drilled into the skull. A gold or silver ring or bar is then passed through this hole. Apparently the massaging of the jewelry on the brain can create a feeling of euphoria on demand akin to some heavy duty narcotics.
